Student-Athlete Medical Forms

2023-24 Academic Year Information

Each year, all student-athletes must complete and submit the requirements listed below to the athletic training room by the following deadlines:            

July 11 - Football

July 21 –  All other fall sport student athletes (Cross Country, Field Hockey, Men’s and Women’s Soccer, Volleyball, Men’s & Women’s Golf)

August 11 – Spring sports student athletes (Baseball, Lacrosse, Softball), and Swimming & Diving

August 11 – Winter sport student athletes (Men’s and Women’s Basketball, Ice Hockey, Track and Field)

January 5, 2024- Spring semester transfer students

These forms MUST be completed before you are eligible to practice or compete in intercollegiate athletics:

All medical information will be stored on our secured ATS (Athletic Trainer System) Web Portal

1. PHYSICAL:  Per NCAA policy, a physical completed within six months of your first practice must be submitted each year. The physical and immunizations must be uploaded to the ATS web portal.

2. SICKLE CELL

The NCAA requires student-athletes to confirm their sickle cell trait status (SCTS). Student-athletes who are positive for sickle cell trait will be allowed to participate in intercollegiate athletics. This does NOT prohibit the student-athlete from participation. New student athletes must upload a copy of their newborn sickle cell test results with your physical onto the ATS online portal. 3. ADHD

The NCAA requires student athletes who are taking medication for ADHD to click on the link below and submit the required information.

5. A hard copy of your physical and immunizations must also be mailed or faxed (413-572-5545) to Westfield State University Health Services, Scanlon Hall, 577 Western Avenue, Westfield, Ma. 01086 .
